APS Middle School Sports

 

The following items are mandatory for participation in middle school girls volleyball, boys and girls basketball and boys and girls track & field for the 2021-2022 school year.

 

It is mandatory for each student to have the completed forms prior to tryout dates. Students missing any part of the packet will not be eligible to participate in tryouts. 

 

Tentative Tryout start dates:

Volleyball: Week of August 30th

Boys Basketball: mid October

Girls Basketball: January 2022

Track & Field: TBD

 

Physical Examination Requirements

  • A physical examination using the NMAA physical form dated on or after April 1, 2021 is required for student participation, including tryouts - the  NMAA physical form is one part of the required NMAA physical packet - ALL parts of the packet must be completed and are required for student participation, including tryouts

  • Completed physical forms require an approved health care professional's signature and indication on the form verifying if the student is medically eligible for participation in sports without restriction  

  • The NMAA physical packet includes the following forms that must be completed, signed and returned prior to participation, including tryouts:

    • Consent to Treat Form 

    • Concussion in Sports - A Fact Sheet for Athletes and Parents handout including an acknowledgement form that must be completed 

  • Documents requesting a parent signature must be signed by either a parent or legal guardian of the student 

  • A copy of emergency contact information and a signed Consent to Treat Form for each participating student in an athletic program must be provided to and in the possession of coaches when working with a team (e.g. practice, contests, meetings, etc.).

Athletic Participation Consent Form 

  • The completion of the Athletic Participation Consent Form is required of all students prior to participation, including tryouts 

  • This document must be reviewed by a parent/legal guardian and student and completed with requested initials prior to participation in athletics, including tryouts.

  • The form is available in English and Spanish on the APS Athletic Department website; refer to the link provided

NMAA COVID-19 Medical Clearance 

  • Students who have contracted COVID-19 will require medical clearance prior to participation, including tryouts, using the NMAA COVID-19 Medical Clearance Form

  • Completed forms require an approved health care professional's signature and indication on the form verifying if the student is cleared for activity 

  • An incomplete form regardless the section shall not be accepted and student participation shall be prohibited until completion  

  • Included with the NMAA COVID-19 Medical Clearance Form is a handout providing recommended procedures for returning to play after contracting COVID-19 - procedures for return to play progression are recommended but not required - the medical clearance form is required
